Perinatal & Reproductive Psychiatry

Perinatal and reproductive psychiatry focuses on mental health during times of hormonal and life transition - particularly during pregnancy, the postpartum period, and other stages of reproductive change. These periods can bring profound emotional, physical, and psychological shifts, and mental health support can play a critical role in helping individuals navigate them safely and confidently.

At Empire State Psychiatry, we treat a wide range of conditions related to reproductive mental health, including prenatal and postpartum depression, postpartum anxiety, intrusive or obsessive thoughts after childbirth, mood changes related to fertility treatment, and psychiatric concerns related to miscarriage, pregnancy loss, or termination. We also help patients manage existing mental health conditions while planning for pregnancy or during pregnancy and breastfeeding.

Our approach is collaborative, evidence-based, and individualized. We work closely with patients - and when appropriate, their obstetricians, fertility specialists, and primary care physicians - to carefully weigh treatment options, including psychotherapy and medication, with careful attention to the well-being of both parent and baby.

Signs and Symptoms of Postpartum Depression

It is common for new parents to experience emotional changes after childbirth. However, postpartum depression is more persistent and severe than the short-lived “baby blues.” Symptoms can develop anytime during the first year after delivery and may include:

  • Persistent sadness, emptiness, or hopelessness
  • Frequent crying or feeling overwhelmed
  • Anxiety, excessive worry, or panic
  • Difficulty bonding with the baby
  • Irritability or anger
  • Loss of interest or pleasure in activities
  • Changes in sleep or appetite beyond the normal disruptions of caring for a newborn
  • Feelings of guilt, shame, or inadequacy as a parent
  • Trouble concentrating or making decisions
  • Intrusive or distressing thoughts

Postpartum depression is common and highly treatable. If you or someone you love is experiencing these symptoms, professional support can make a meaningful difference. At Empire State Psychiatry, we provide compassionate, specialized care to help parents recover and thrive during the postpartum period.

Signs and Symptoms of Premenstrual Dysphoric Disorder (PMDD)

Premenstrual Dysphoric Disorder (PMDD) is a severe, hormone-related mood disorder that occurs during the luteal phase of the menstrual cycle, typically in the one to two weeks before a period begins. Symptoms improve or resolve shortly after menstruation starts. PMDD can significantly affect daily functioning, relationships, and quality of life.

Common symptoms include:

  • Severe mood swings
  • Marked irritability or anger
  • Depressed mood or feelings of hopelessness
  • Anxiety, tension, or feeling “on edge”
  • Sensitivity to rejection or interpersonal conflict
  • Difficulty concentrating
  • Fatigue or low energy
  • Changes in appetite or food cravings
  • Sleep disturbances
  • Physical symptoms such as bloating, breast tenderness, headaches, or joint pain

PMDD is a medical condition - not simply “bad PMS.” With proper evaluation and treatment, symptoms can often be significantly improved. Our clinicians work with patients to develop personalized treatment plans that may include medication, therapy, and cycle-based strategies to stabilize mood and improve quality of life.
